Monthly Payment Package Plan (“MPPP”)

Synopsis of the Contractual Plan:

  • Plan May be Redeemed by Various Clients; Share the Sessions with Family and Friends
  • Relaxation or Medical Massage
  • Must Schedule All Sessions (12) Within 1 Year
  • Choose Your Own Intervals for Scheduling Appointments
  • Hourly rate is $50 with a Minimum Payment of $50 (Charge) or $48 (Cash or Check) per Month. We offer a $2 Discount for Cash or Check Payments. Sales Tax is Included.
  • Payment for First and Last Appointments (Total $100/$96) is Required at First Appointment, then 10 Monthly Payments of $50/$48 Each to Cover a Total of 12 One-Hour Sessions
  • You Have Up to 30 Days to Make Next Payment or You Lose Eligibility for Plan
  • No Penalties; May Drop Out of Plan at Any Time but There is No Refund of Payment for Final Session
  • No Automatic Payment Withdrawals
  • Your Choice of Participating Therapists, Excluding Carol
Terms:

  • In order to remain in the Monthly Payment Package Plan (“MPPP”), you must submit your payments in a timely manner by the payment due date (PDD). Whatever day of the month you choose to start your package plan will be your payment due date (PDD) for the following 10 monthly payments.
  • If you miss a PDD, you have a maximum 15-day extension period to make the payment before losing the option to use this plan. You may not opt for this plan again in the future if you default.
  • You may schedule your appointments any time within the 12 months after your Start Date as long as the corresponding payment(s) has been made ahead of time or is made during the appointment.
  • Payments must be in our hands if you deliver them personally OR postmarked by the payment due date.
  • The option of more than 12 sessions within this plan within 12 months must be approved by CMT and may include additional stipulations.
  • If you send another person to use one of your 12 sessions, we must get a verbal or written notification from you.
  • Our regular gift certificates may not be issued in connection with your MPPP.
